Formative Evaluation – determination of risk, target audience, intervention, and program objectives, and the methods used for the evaluation. Little to no description of risk, target audience, intervention, program objectives, or methods used. Some, but not all, of the risk, target audience, intervention, program objectives, and methods used are described. Risk, target audience, intervention, and program objectives are sound and all are included but the evaluation method is not explained. Risk, target audience, intervention, and program objectives are sound and all are included as well as the methods used to make the determinations.
Process Evaluation – analysis of the program’s development, strategy, and implementation; how they addressed the findings of the formative evaluation. Actions taken are not documented or none are measurable. Some, but not all, of the actions taken, resources used, and accomplishments are measurable or well documented. Actions taken, resources used, and accomplishments are measurable and are documented so that another agency can duplicate the program. The reasoning, actions, barriers, solutions, resources, and accomplishments are measurable and documented so that another agency can duplicate the program.
Impact Evaluation – changes to knowledge, behavior, resources, response, etc. No changes were made or identified. Changes were partially documented, though perhaps not in a very measurable way. Changes were documented and measurable. Changes were well-documented, measurable, and significant in leading to risk reduction.
Outcome Evaluation – long-term proof of risk reduction. There is evidence of effectiveness for change or risk reduction.

No known outcomes, no outcomes address long-term reduction to injury, death, or economic loss, or too little time for long-term evaluation.

Knowledge-only change in plan, outcome, and/or impacts.

Weak outcomes but they are associated with common risk reduction long-term goals.

Accomplished goals that are associated with risk reduction.

Proof of long-term reduction to injury, death, or economic losses. Perhaps not enough time has passed but the initial numbers are promising.

Addressed positive changes to behavior, environment, or process.

Strong proof of long-term reduction to injury, death, or economic losses.

Proved a reduction of loss of life, injury, or resource use.
